Fill out and submit this form to register for the Take the Ice Program on Fridays at 5:30pm, Glen Abbey Arena. Please note, upon receipt of this form the ORHC will use this information to create a Hockey Canada Player’s Profile for your child.

Refund Policy: Please be aware that registrations may be cancelled and refunded only prior to receiving the equipment package. Once the equipment package has been issued, the registration is considered FINAL and no refunds will be provided.
